F2F Learning Cohort Schedule and Transportation Information
We will begin F2F instruction for those that selected to return. Students will report two days each week on designated days based on their cohort. 6th graders will begin reporting on Tuesday, March 9, 2021 and 7th/8th graders will begin reporting on Monday, March 15, 2021. Please see the information below for the days your student will be reporting as well as transportation and uniform requirements. The instructional day id from 8:50am to 3:50pm. Car riders need to be dropped off between 8am and 8:15am and must be picked up by 4:15pm. We will not have After School Extended Daycare for the remainder of the year.

*If you feel there is a mistake and your student should either be removed from or added to the F2F Cohorts please email the Principal Rhodes by March 4, 2021.
 
Uniform Expectations for Students Returning to F2F instruction!
 
Commitment/Rejection form for the 21-22 school year

6th and 7th Grade Parents as we plan, it is vital that we know how many students will be returning to Champion for the 2021-22 school year. The link below will allow you to indicate your intent to remain at Champion Theme Middle School. Your prompt response is appreciated to determine the number of seats we will have available for new applicants. Please follow the instructions below and complete by March 8, 2021:

1. Complete the intent to return survey to let us know if your student will be returning or not: CLICK HERE to complete the intent to return survey.
2. If you answered "NO" to returning you will also need to print and email a signed copy of the commitment/rejection form to: annazette_taylor@dekalbschoolsga.org. To download the commitment/rejection form please CLICK HERE.
3. For those who answered "YES" to returning, please DO NOT email a signed copy.
 
Chromebook Inventory & Wellness Check
The Division of Information Technology is facilitating a system Inventory and Device Wellness Check for all student Chromebook devices as we prepare for the Georgia Milestone. They are asking all schools to account for the Chromebook devices assigned to students at the school level since these devices were not on-site during IT/Vendor visits. In order to facilitate this, we need for you to physically locate your students school issued Chromebook in order to retrieve some information and complete the survey at the link below. If your student was never issued a Chromebook or it is out for repair we still need you to complete it. Thanks in advance.

Virtual Learning Support
Parents if you have a Chromebook that is still in repair, please call the DeKalb County School District Help Desk at 678-676-1188 for an update. You may also use the links below for other assistance.

Required PTSA Membership (MemberHub)
PTSA membership can now be purchased through MemberHub. Please note that joining PTSA is a requirement for current enrollment and continued enrollment in Champion Theme Middle School. We will use this platform to verify your membership and it will be the only method of accepting your required membership payment. 
  1. Please click the following link to join MemberHub for the first time: https://championmsptsa.memberhub.com/join/u286xa
  2. Once you sign up you will receive a confirmation email shortly thereafter to complete the process. Once you are in MemberHub, all you have to do is click "shop" at the top.
  3. The only item that you will be able to purchase through MemberHub at this time is your PTSA membership. Your membership will be verified through MemberHub and you will receive an email that will allow you to complete the process, download your membership card and your online receipt.
